Public Health Nurses

Public Health Nurses serve New Haven’s culturally diverse urban community by promoting and encouraging a healthy lifestyle for its residents. They run the Health and Wellness Center, lead community health programs, and work inside New Haven Public Schools.

School Health Requirements

2025-2026 School Year

Students entering Pre-Kindergarten, Kindergarten, 7th or 10th grade: must receive a physical exam and required vaccines or they cannot start school in August 2025. Records must be dated on or after September 1, 2024.

Pre-Kindergarten 3 & 4-year-olds: must receive a flu vaccine by December 31, 2025 or they cannot return to school in January 2026.

You may also drop off a printed copy to your school nurse or to the New Haven Health Department at 424 Chapel Street, Monday-Friday, 9:00am-5:00pm. For any questions, call 203-946-8181.

Health information for school entry is based on state laws. If your child had a physical out of state, everything on the Connecticut Health Assessment Form must be included.

Vaccinations

See required vaccination guides below for students in preschool through high school.

To get an exemption from vaccination requirements for medical reasons, this form is required. 

CT state law no longer allows for religious exemptions for childhood vaccination requirements for schools.

Students with Food Allergies: New for 2024-2025

All students with a food allergy or who need a meal modification for a medical reason must have a form filled out and signed by their doctor.

Please return ASAP to your school nurse.

School Health Centers can simplify care for your child

Student Health Records

Student immunization records and Cumulative Health Records (CHR) are available upon request to students who go to or have graduated from New Haven Public Schools. Requests must be submitted using this form.

Any completed forms we receive will be processed within 7-10 business days.

When Should Your Child Take A Sick Day?

Guidelines for school attendance are updated based on the latest medical advice.

  • If your child has an illness that can be spread to others through direct or indirect contact, please do not send them to school until they have been cleared by their healthcare provider or they no longer have symptoms.
  • Please notify the school when your child is ill.
  • If you’re having trouble deciding whether to keep your child home from school when ill, contact the school nurse’s office.

School Health Forms

It is important to notify your school nurse if your child has a food allergy, diabetes, asthma, other medical condition, or if ANY medication is needed during school hours. Specific forms may be required.

School Health Screenings

Hearing, vision and scoliosis screenings for students are required by state law. Screenings are free and done during the school day.

For students in Kindergarten, 1st, 3rd, 4th, and 5th grades only
