Do I need to make an appointment?
Appointments are needed for school physicals, childhood immunizations, and tuberculosis testing. All other services are walk-in. Call 203-946-8181 to make an appointment.
How much do services cost?
Our staff work within your budget to try to make sure you get the care you need. Payment for services is due at the time of the visit. If you have health insurance, please bring your card and photo ID. Without insurance, there is a minimum fee of $20.
I don’t have health insurance. Can I still come to the Health and Wellness Center?
Yes! The Health and Wellness Center works hard to provide affordable health care for everyone, whether or not you have insurance. Our staff work within your budget to try to make sure you get the care you need.
If you don’t have health insurance right now, you may qualify for other programs that can help pay for healthcare services. Go to Access Health CT or call 211 for information about signing up for health insurance. If you have questions about getting health care without insurance, call us at 203-946-8181.
Do I need to be present for my child’s appointment?
A parent or guardian must give permission for school physicals and immunizations if the patient is under 17 years of age. If a parent cannot be present, a signed permission form must be provided.

Is there bilingual staff available?
Yes, we have staff who speak both English and Spanish. We can also arrange for interpreters to be available over the phone for other languages.
