A convenient way to order copies of your vital records is online through VitalChek. VitalChek is an authorized online vital records ordering service for the City of New Haven. There are additional fees associated with this service. Please note, VitalChek DOES accept personal checks. We, the Office of Vital Statistics, DO NOT accept personal checks.

Birth Certificate

Have the Required Information:

  • Full name at birth
  • Date of birth
  • Both parents’ full names, including mother’s maiden name
  • Parents’ place of birth (state or country)
  • Daytime phone number
  • A valid driver’s license, or two of the following, one of which must have a photo:
    • Social Security Card
    • Employment ID
    • State or City ID
  • If you do not have any of the IDs listed above, please provide copies of at least two recent bills which show your name and current address.

Print and fill out the appropriate form below and bring it with you to the Office of Vital Statistics:

Payment Information
  • Copies are $20.00 each for full size; $15.00 each for wallet size; and $65 each for an original birth certificate of adopted persons.
  • All fees for documents are payable by cash, credit, or money order made payable to the Office of Vital Statistics.
  • We do not accept personal checks.

If you are applying for a certificate by mail, you must send a letter stating what type of certificate you are requesting as well as the required information, identification and forms listed below.

Please make sure to include a return address.

Marriage License

If a couple is planning to marry in New Haven, they must obtain a marriage license from the City of New Haven’s Office of Vital Statistics. The marriage license is issued to ensure that the couple is eligible to be married.

A marriage license issued from the New Haven Office of Vital Statistics is valid only for 65 days from the date it is initially issued to those who are being wed in the City of New Haven. You must be at least 18 years of age to apply. Both parties must be present in order to receive the license.

We encourage everyone to submit their marriage application online and schedule an in-person appointment, although only New Haven residents (at least one spouse must have a New Haven address) will continue to be allowed walk-in, same-day service to apply for a marriage license.

The Office of Vital Statistics is open Monday-Friday, 9:00am-4:30pm, and 9:00am-2:00pm on the first Wednesday of the month.

Have the following required information when arriving to the Office of Vital Statistics:

  • Both parties must have one of the acceptable forms of identification (the IDs must include Date of Birth, Signature, and Expiration Date):
    • Current State ID issued by DMV
    • Current motor vehicle ID issued by DMV (with the correct address)
    • A valid passport
    • Current Military ID
  • Residents who do not complete an online application must show a valid government-issued ID that includes a New Haven address.
Payment Information
  • The fee is $50.00 for a marriage license.
  • All fees for documents are payable by cash, credit, or money order. Money orders should be made payable to the Office of Vital Statistics.
  • We do not accept personal checks.

Officiant:

  • A listing of Local Justices of the Peace can be provided by the City Clerk’s Office upon request. The City Clerk’s Office is located on the 2nd floor of 200 Orange St.
  • The Office of Vital Statistics will not provide an officiant to perform a marriage in City Hall.
  • If you would like to have a marriage ceremony in City Hall, you must bring an officiant with you to perform the ceremony.

Legal changes of name are only available in-person through New Haven’s Office of Vital Statistics and only if the individual was born in New Haven. In order to complete a legal change of name, you must do the following:

  • Bring in an affidavit (letter) stating that you are requesting that the Office of Vital Statistics change your birth name to the new legal name
  • Have the affidavit (letter) notarized
  • Provide the original document (with seal) from the court where your legal change of name was granted
  • Provide one of the acceptable forms of identification (ID):
    • Current State ID issued by DMV
    • Current motor vehicle ID issued by DMV (with the correct address)
    • A valid passport (copy of page with photo and expiration date listed)
    • Current Military ID
  • If you do not have any of these IDs, you will need any two of the following documents:
